$100.00 non-refundable assessment fee to be paid when scheduling your assessment
A current photo of applicant (optional)
Copy of current immunization record
Copy of birth certificate or other proof of age (required for prospective kindergarten students)
Confidential recommendation form (required for prospective 1st-8th grade students)**
